Three forms regarding allergy injections given at the Henry Wise Wellness Center must be read/completed as needed and on file prior to allergy injections.

1. Allergy injection information sheet

Student must read, complete and return the form to the Henry Wise Wellness Center.

3. Physician information sheet

The physician (allergist) must read the attached letter, complete the information sheet, and return it to the Henry Wise Wellness Center in order that you may receive your allergy injections next semester.

It is imperative that you bring paperwork and allergy medication to the health center at least two days prior to the day your receive your first allergy injection at the center. If you or your allergist have any questions or concerns, please contact the Welness Center at 301-860-4170.
