The Major Change Form or “Declaration of Major Form” communicates a major change or a major[1] to the Office of the Registrar. Students are required to complete the form and acquire the signature of the academic department’s chairperson to communicate to the academic department that the student plans to join their academic community or group. Once the academic chairperson signs the form in the appropriate location[2], the student delivers the form to the Office of the Registrar for processing. All students are encouraged to maintain a copy of the Major Change Form.

[1] All undeclared majors must choose a major at the end of their freshman year or at the beginning of their sophomore year.

[2] The form has two places for the academic chairperson to sign; where the chairperson signs will determine the action in the Registrar's Office.
