• View Institutional Review Board Proposal Submission Form 

The BSU Institutional Review Board (IRB) utilizes a Proposal Submission Form that is required for protocol review. You may download the form using either of two formats: MS Word or a PDF file. The primary investigator shall bear final responsibility for submitting all forms required for review under IRB procedures. It is the responsibility of the investigator to use the most current version of the form needed. Do not create your own form as that may result to the rejection of your application.
