Complete this request form for each textbook you need converted.

Please fill out this form in its entirety. Make sure you complete the Accessible Text Agreement. (One Accessible Text Agreement will suffice for all your book(s) request(s). Due to the amount of time required to provide this accommodation and high volume of requests, you should submit your request at least three weeks before the Semester begins and to ensure that your request is completed in a timely manner. Requests will be processed in the order in which they were received. It may take 15 days for your request(s) to be completed.
